Changing Your Legal Name
Students who marry, divorce or by court order change their name and wish to change the name on their permanent academic record must do so with the Registrar's Office. The student should submit a Change of Legal Name form to the Registrar's Office. The request must be accompanied by a social security card bearing the new name.
If you have not requested a change of legal name with the Social Security Administration, please apply for a new social security card with the nearest Social Security Office. Information on how to apply for, replace, or change your name on your social security card is available online at
NOTE: Refer to the Preferred or Chosen Name, Gender Identity, and Pronoun Policy for changes not requiring a legal name change.

The 91社区 Information Technology Department assigns an email account to every newly enrolled student which is maintained throughout your time as a student. You can get your 91社区 email address through My91社区. If you have any questions about your 91社区 email account, contact the Information Technology Help Desk at 812-465-1080.
You must use your 91社区 email for all university communications. There are times when only your university email address can be used to gain access to some University services such as My91社区, Blackboard, and library databases from off campus locations. Students must have an established an email account before the first class meeting. Students are responsible for checking 91社区 email accounts daily while enrolled in coursework.

Personal travel is required of students for all clinical courses. Liability for driving to and from clinical sites falls on the owner of the vehicle. Automotive insurance is required. Information concerning registration of your car and parking at clinical agency sites will be provided by the clinical agency.
Extensive library services are available to students at 91社区's David L. Rice Library.
Services include interlibrary loans, research information, computer access, and copying/printing services. Students should contact the library (812) 465-7127 or through the library homepage for a current listing of library hours.
Students are expected to have healthcare insurance while enrolled in the program. The student is responsible for all costs, including any costs associated with any type of clinical exposure, while enrolled in the program.
Students are required to follow the College Infection Control Policies required for health exams, immunization and tuberculosis testing policies. These records are tracked through student enrollment in the third-party repository.
Pregnancy and Change in Health Status
Student pregnancy or a change in health status is to be reported to the course coordinator. A physician's consent is required for students to continue clinical agency experiences. After surgery or other hospitalization, a physician's release to return to clinical is required.

Students who are injured in the clinical setting are to report the incident IMMEDIATELY to their instructor and complete an agency and School incident report. The College incident report will be filed in the Dean's office.
Students are responsible to remove themselves from clinical environments where they perceive risk for harm. In this situation, clinical faculty should be notified the same day the incident occurred.
Students who exposed to blood and body fluids in the clinical setting are to report the incident IMMEDIATELY to their instructor and complete an agency and School incident report. The College incident report will be filed in the Dean's office.

Nurses as role models and providers of care must avoid lifestyle factors associated with disease. Smoking is prohibited in all clinical environments and all 91社区 campus buildings. Students are not allowed to smoke, use E-cigarettes, vapor nicotine or oral tobacco in any clinical facility while in the clinical environment or on 91社区 campuses.
Courses and clinical assignments may be scheduled with relative short notice during the semester. This allows us to provide you with optimal learning experience. We cannot guarantee absence of conflict with your work schedule.

Sigma Theta Tau International/Omicron Psi Chapter
The Nursing Honor Society at the 91社区 provides leadership and research development for eligible nursing students and professionals. Its purposes are to:

- Recognize the development of leadership qualities
- Foster high professional standards
- Encourage creative work
- Strengthen commitment to the ideals and purposes of the profession
Students in the graduate program are required to achieve a 3.5 GPA on a 4.0 scale or its equivalent, and to have completed one-fourth of the graduate program of study to be considered for membership.
Program documents, including those verifying program completion and required clinical rotations, will be maintained by the college. Personal background information will be stored in the student鈥檚 third-party repository account.
