Health Service Program Requirements
To be considered for admission to the Health Services program, students must complete the following requirements:
- Acceptance to the 91社区听
- To apply to the University, you must complete an听admission application with 91社区 Admission. Health Services applications will not be considered for admission until the student has applied to and been accepted to the University.
- High school diploma or GED
- Decision to pursue a degree completion, Health Services Generalist track, or Health Services Specialty track
- If pursuing the Health Services Specialty track
- Students must complete the prerequisite courses.
- There are no pre-requisite courses to declare the Generalist Track.
- Have a minimum cumulative grade point average of 2.75 (4.0 scale).
- There is no GPA requirement to declare the Generalist Track.
- Students must complete the prerequisite courses.
- If pursuing the Health Services Specialty track
The Bachelor of Science in Health Administration (BSHA) is available should the student prefer a degree with a more healthcare administration focus.

How to Apply for a Specialty Track
Make sure you meet the prerequisites and admission requirements as stated above before applying.
- Submit an application to the 91社区 Health Services Program
- Applications will be reviewed on a rolling basis.
- Once you are accepted into the Health Services program, you will need to complete the following Castle Branch requirements before applying for an HP480 Internship.
Castle Branch Information
The College of Nursing and Health Professions uses Castle Branch for required criminal background checks, drug screen, and tracking of immunizations and other required documents.
Click on the link provided to start your Castle Branch profile:
- Your profile through Castle Branch must be completed prior to applying for an internship. You should start the process NOW!! Some requirements can take several months to complete.
- Be sure to use your 91社区 email address. Automated reminders will be emailed to you when your documentation is about to expire OR if your submitted documentation is incomplete or incorrect. If you use yahoo, hotmail or any other email accounts, you may not receive these reminders or notifications. It is essential that you use your eagles.usi.edu account.
- During the registration process, you will be asked for a package code. Please select from below.
- UF02 and UX09 requires you to go to LabCorp (925 S. Green River Rd., Evansville, IN 鈥 Phone: (812-477-5518) for your drug test (minimum 10 panels).
- You will receive an email from CastleBranch with an attachment of the Registration Form that you will need to take with you to LabCorp.
- Health Services or Health Administration students: Use UF02 code
- Health Informatics student: Use UX09 code听

How to Apply for the Generalist Track
There is no application process to declare the Generalist Track. Students may declare the major upon entering the university or complete a change of major form at any point in time.
How to Apply for Degree Completion
Students who have a healthcare clinical license (where college credit was not available) may qualify for the degree completion option. Applicants must meet admission requirements for the University and have a minimum of a 2.0 GPA to make application to the Health Service Program Degree Completion an option.
