During the Benefits Open Enrollment period as a benefit eligible employee, you have the opportunity to evaluate your insurance needs and make changes in your benefits for the following plan year. The 2025 Open Enrollment period is November 4, through November 15, 2024.

  • Benefits Bash: This event includes the benefits team and providers available for questions in a fun and festive environment. Providers will have materials and giveaways and there will be prizes for attendees.
  • Presentations: Various vendors will host presentations to provide education about their products. Some will be in person during our Benefits Bash and others will be online.
  • Additional meetings: the benefits team will make themselves available during the open enrollment period with the following times:听
    • November 15, 2024 9:00 a.m. - 3:00 p.m.
    • Email benefits@usi.edu for individual meetings

In 2025, the Core PPO Plan will see a 5% premium increase, the Surest PPO Plan will have a 2% premium increase, while the HSA PPO Plan will not experience any increase in premiums. These variations are due to the differing costs incurred by each plan over the year.听听

  • UMR Core PPO Plan: Deductibles, co-pays, coinsurance and out鈥搊f-pocket maximums for Core PPO plan remain unchanged.
  • Surest PPO Plan: Each year, Surest evaluates providers and adjusts co-pays based on their performance. To find the co-pays for 2025, members can visit the member or app. If not a member yet, please visit the pre-member website at for more information.听
  • UMR HSA PPO Plan: Coinsurance and out-of-pocket maximums remain unchanged. Due to IRS requirements, deductibles will increase by $100 to $3,300 for individual coverage, and by $200 to $6,600 for family coverage.
  • Prescription Plan: The prescription plan will still be managed by CVS Caremark, but there will be a new plan number and a 30% copay for specialty brand medications. This change enables the University to introduce a Specialty Co-pay Assistance Program, which allows members to obtain co-pay assistance directly from the drug manufacturer. In most cases, this means members will pay less than their previous copay, and at a minimum, their costs will remain the same.
  • WellBridge Surgical: If the time comes and a member needs surgery, WellBridge Surgical is available to Core and Surest members at no member cost and HSA members at reduced pricing. Learn More about Wellbridge Surgical in the Benefit Enrollment Guide.
  • Dental: There are no premium increases or plan design changes to the Dental plan. Paramount Dental will continue to be the provider.
  • Vision: There are no premium increases or plan design changes to the vision plan.鈥疉nthem Blue View Vision will continue to be the provider.听
  • Employee Assistance Program (EAP) - TimelyCare will continue as the Employee Assistance Program (EAP) provider. TimelyCare provides 24/7 access to mental health providers and six scheduled counseling sessions per year.
  • Health and Dependent Care FSA - Voya will continue to serve as the administrator for both the Health and Dependent Care (childcare) Flexible Spending Accounts.听An individual can contribute up to $3,300 into a Health FSA and up to $5,000 into a Dependent Care FSA in 2025.
  • IRS HSA limits for 2025: An individual can contribute $4,300 and a family can contribute up to $8,550 in 2025. This includes the University鈥檚 contribution that remains at $750 for individuals and $1,500 for family coverage.听
  • You must complete your open enrollment through the online benefits platform.
  • When you complete your enrollment you will be entered into our drawing for awesome prizes!
  • Enter or update your beneficiaries in the online benefits platform.
  • Enroll in, drop or change coverage in the medical, dental or vision insurance plans, including adding or dropping dependents
  • Enroll or re-enroll in the health care or dependent care flexible savings account (FSA)
    • Eligible employees听must enroll or decline these听plans annually
  • Participants of the HSA plan:
    • Eligible employees must enroll or re-enroll in HSA annually
    • 91社区 contributions will automatically听renew based on plan election
  • Enroll in, drop, or change coverage in the additional life or short-term disability plans
  • Enroll in, drop, or change coverage in the voluntary accident, hospital or critical illness plans
  • Attend Presentations (schedule)
  • Enroll online by November 15, 2024!

  1. Access the Benefits Portal online by visiting
  2. Once you have logged in and have clicked on the Click Here icon, select Document Center. You will be able to upload documents directly to your pending documentation requests by selecting Upload a Document.
  1. Search for 鈥淏enefitfocus鈥 in the App Store or Google Play and download to your mobile device. Use Company ID 91社区 to activate the app.
  2. Adding Documents
    • Log in to the Mobile App using your My91社区 credentials
    • From the home screen, select the icon on the bottom labeled Account
    • From the Account screen, select Documents
    • Select the documentation request and then Document Required
    • Tap the plus sign and take a photo or choose an existing photo of the document you want to add
    • Enter a Document Name
    • Select a category for the document
    • Select Save Document
  3. Tap OK on the confirmation screen. You are returned to the Documents screen

Deductibles - refer to the amount a member must pay before the plan starts paying.

Co-insurance - refer to the percentage a member pays until the out-of-pocket maximum is reached.

Co-pays - refer to the amount a person pays for a specific service, like office or urgent care visit.

Out-of-Pocket Maximum - refers to the maximum amount of eligible claims a member will be required to pay.听

Employees will need to log into the Benefitfocus System and elect their new medical insurance coverage as well as review their other coverages.

Be sure to choose all of the dependents you want to be covered. Print or save your benefit summary at the end of your enrollment.听听

To find out if your doctor is in-network for the UMR and Surest plans, go to , click on 鈥渇ind a network鈥, 鈥淢edical Directory鈥 鈥淓mployer and Individual plans鈥 and then choose the Choice Plus network.

UnitedHealthcare has the largest proprietary provider network with over 1.2 million providers, over 5,400 hospitals, and 2,300 convenient care clinics.听

To view claims information members will go to for听the Core or HSA Plans or for the Surest plan.听听

After Open Enrollment, a file is sent to the providers and cards are sent out with the goal of arriving before 12/31/2024. Please be sure to fill any essential prescriptions prior to the end of the year to avoid any last-minute scramble. With a change like this one must remember we are relying on the mail to run on time and unforeseen issues can arise. 听

  • Core and HSA Plans 鈥 The cards for the UMR Core and HSA Plans will have the UMR and UnitedHealthcare logos and include the CVS Caremark Prescription information.
  • Surest Plan 鈥 The cards for the Surest plan will have the Surest and UnitedHealthcare logos and will not include the CVS Caremark information. Members will get a separate card for the Prescription coverage.

Surest is an employer-sponsored health plan that gives members the tools to see cost and coverage information before the point of care. Members receive quality medical coverage with access to the broad, national UnitedHealthcare and Optum Behavioral Health networks.听
