This section covers the various aspects involved with becoming an Authorized Driver for the 91社区 (91社区). It also has a link to the 91社区 Vehicle Safety Policy, which is designed to ensure that only Authorized Drivers use 91社区 vehicles, sets expectations for the safe use of 91社区 vehicles, and establishes accident reporting requirements.

Unauthorized drivers are not allowed to drive on behalf of the university.

All employees, students, student workers, and volunteers whose job responsibilities or requested duties involve driving on 91社区 Business, must meet the requirements of the Vehicle Safety Policy found in the University Handbook.

91社区 B91社区NESS

91社区 Business means activities that further the mission of the University. This applies to local, in-state, out of state, or assigned job duties (on or off campus), regardless of the frequency or distance of the approved 91社区 business trip or activity. Examples include conferences, meetings, campus tours, supervised student clinicals, field trips, academic competitions, recruiting, etc.

IMPORTANT: Based on the criteria in the Vehicle Safety Policy and the provisions of the University Handbook, all employee(s), student(s), or volunteer(s) who drive on approved 91社区 business, must be an Authorized Driver. Unauthorized drivers are not allowed to drive on behalf of the University.

91社区 VEHICLE(S) ARE DEFINED AS:

  • 91社区 owned, leased, loaned, and rented licensed motor vehicles
  • 91社区 owned, leased, or loaned golf/utility carts (licensed or unlicensed)
  • Personal vehicle when operated on 91社区 Business

IMPORTANT听鈥 Please read section听Automobile Liability Coverage When Driving on 91社区 Business

91社区 DRIVERS ARE DEFINED AS:

"Potential" Drivers: Employees, students, or volunteers who may drive for 91社区 Business

  • Must be 18 years of age or older and have a valid driver鈥檚 license
  • Employee (faculty, staff) of 91社区 whose job description requires driving as a condition of employment.
  • Employee of 91社区 on official business travel
  • 91社区 student (undergraduate, graduate) working in a paid position on campus who is either traveling on valid 91社区 business or whose job description requires driving as a condition of employment.
  • 91社区 student, at the request of their professor, traveling on approved 91社区 Business
  • Another individual who may be permitted on an occasional basis to drive on valid 91社区 business.

91社区 "Authorized" Driver: Employees, students, or volunteers who have completed the Driver Requirements in the Vehicle Safety Policy. Authorized Drivers may drive on 91社区 Business

"Conditional" Driver: An Authorized Driver who must comply with assigned conditions, per Appendix B of the Vehicle Safety Policy, to continue to drive a 91社区 vehicle

Risk Management is responsible for the screening and authorizing of each Potential Driver for 91社区. A Potential Driver means any new or established employees, students, student workers, or volunteers who drive for 91社区 as part of their job responsibilities or requested duties. Based on the criteria in the听 (VSP) and the provisions of the University Handbook, all employees, students, or volunteers who drive on 91社区 business, must be Authorized Driver.听

91社区 requires all persons who drive on behalf of 91社区 to become Authorized Drivers with Risk Management. Authorized Drivers must have the following information on file with Risk Management: A Potential Driver Acknowledgement form, completed the Lezage Training Module for Defensive Driving (and Golf/Utility Cart and 15 Passenger Van training if specified by supervisor), and a Motor Vehicle Report.

Supervisor Action

Supervisors should make all Potential Drivers (PD) aware of the 91社区 Vehicle Safety Policy.

Supervisors must complete the for each employee, student, or 91社区 volunteer that may drive on behalf of 91社区.

Additional Information for Supervisors:

    • Once Risk Management receives the Supervisor PD Notification form, an email will be sent to the Potential Driver with 鈥淧otential Driver Instructions鈥 and access codes.
    • Supervisors/Departments are required to submit the 91社区 Risk Management Self-Disclosure Form, if they become aware that an individual had an accident while driving on University Business, had their driver's license suspended, or if any other event occurs that could affect their driving privileges.
    • The Authorized Driver Database may be found on the O: Drive, under the Risk Management folder.

Authorized Driver Status

The Supervisor and Potential Driver will be notified by Risk Management once all necessary steps are completed and Motor Vehicle Reports (MVR) are reviewed.听听

Driver License

When an Authorized Driver renews their Driver's License, the Authorized Driver should email a copy showing the new expiration date to 91社区1riskmgt@usi.edu or they may send a hard copy in Campus Mail to Risk Management.听

IMPORTANT:听Risk Management does听not keep a record of Authorized Driver Licenses on file. All copies of the renewed Driver's License are destroyed.

Self-Disclosure Form

All Authorized Drivers must disclose a driving offense that could potentially affect the individuals鈥 ability to perform the duties, safely or effectively, in positions that drive a university vehicle in any capacity. Any driving offense, arrest, or conviction must be submitted within seventy鈥恡wo (72) hours by completing the听91社区 Risk Management Self-Disclosure Form. The Disclosure Categories include traffic citations/moving violations, suspension/loss of driver鈥檚 license, and driving while under the influence. Major Offenses include traffic accidents with bodily injury or fatality, traffic accidents involving alcohol/drugs, or other similar charges/convictions.

  • Supervisors/Departments are to report to Risk Management if they become aware that an individual on their driver's list has had an accident while driving on University Business, had their driver's license suspended, or if any other event occurs that could affect their driving privileges.

The is designed to ensure that only qualified Authorized Drivers utilize 91社区 vehicles, set expectations for the safe use of 91社区 vehicles, and establish accident reporting requirements.

The safety of 91社区鈥檚 employees, students, and the public is paramount. This policy is intended to reduce the potential risk of vehicle accidents and to provide safe conditions and equipment for employees, students, campus visitors, and the general public.

University Vehicles听

91社区 carries Automobile Liability on 91社区-owned, leased, loaned, and rental licensed vehicles, including licensed golf/utility carts/trailers. This coverage responds to claims brought by third parties for bodily injury or property damage that arises out of the use of these vehicles. A 91社区 employee, student, or volunteer may drive a personal vehicle on official 91社区 Business.听

However, when driving personal vehicles, insurance coverage is as follows:

  • Personal Vehicle Automobile Liability听-听the Authorized Driver鈥檚 personal automobile insurance is always PRIMARY. 91社区鈥檚 insurance is EXCESS of the Authorized Driver鈥檚 primary limits for damages to a third party.
  • Personal Vehicle Accident Reporting听-听the Authorized Driver must report all accidents to their personal automobile company and notify Risk Management of the accident as soon as feasible.

Rental Vehicles

91社区 carries Automobile Liability on 91社区 rental vehicles. This coverage responds to claims brought by third parties for bodily injury or property damage that arises out of the use of these vehicles. Please note the following:

  • Decline all insurance offered by the rental car company because you are covered by 91社区鈥檚 insurance policy. You will not be reimbursed for the cost of any insurance coverage that you may choose to accept.
  • After signing the rental agreement, write 鈥淭he 91社区鈥 below your signature.
  • Look over the rental vehicle and notify the car rental company of all damages (scratches, stains, markings,)听产别蹿辞谤别听leaving the rental lot.

Vehicle Accident Guidelines

If you are involved in an accident, please follow the guidelines listed here. Complete the information sections as thoroughly as possible.

  • Stop Immediately.
  • Call the appropriate law enforcement agency immediately to investigate:
    • For accidents on campus, call 91社区 Public Safety 812-464-1845 (non-emergency) or 812-492-7777 (emergency).
    • For accidents off-campus, or those involving injuries, call 911.
    • The Authorized Driver must report the accident to their Department Head
  • Do not move the vehicle until the investigating officer gives you permission or local law requires the vehicle to be moved.
  • Obtain medical assistance for anyone injured.
  • Do not accuse others or make any admission of responsibility for the accident. Cooperate fully with the law enforcement authority investigating the accident.
  • Exchange complete information with the other drivers involved. Use the Vehicle Accident Reporting form to record their information. If you are able, please provide pictures of both vehicles (damaged) and the intersection.

Reporting Vehicle Accident and Procedures

The Authorized Driver is required to notify Risk Management of vehicle accidents, accidents involving bodily injury, and accidents involving property damage as reasonably possible following the accident.
